3/10/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in good mood evident by her facial expressions such as a smile on the face and engaging in general conversation with QMHP-C. QMHP-C informed Ashiyah that this session was going to focus on her anger management at which mother reports behaviors last up to 3 days and 5-10 minutes in duration. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C. Ashiyah expressed how she was going to continue to work on her anger and learn from the trouble she has gotten herself in such as her self-inflicting behaviors at which behaviors last about 10 minutes in duration. 1.Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C discussed with Ashiyah methods to controlling her frustrations and learning pro-social behaviors to avoid negative interactions with peers. QMHP-C confronted Ashiyah consistently and firmly when exhibiting an attitude of disrespect and rudeness toward the rights and feelings of others. 2.Intervention: QMHP-C modeled to Ashiyah cooperative and respectful behaviors rather than acting out inappropriately. QMHP-C taught Ashiyah the positive effects of being honest and reliable as it pertains to building interpersonal skills through modeling appropriate behaviors. QMHP-C taught Ashiyah the value of taking full responsibility for her behavior's and learning to admit her wrongdoings and behaviors were reviewed. 1.Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah struggles with positive peer relationships and engaging in healthy interpersonal skills. Ashiyah shut down when confronted about her attitude and disrespectful behaviors. Ashiyah became quiet and slowly began to communicate with QMHP-C. 2.Response: Ashiyah verbalized that she knows how to react to peers but when angry and frustrated she reports that she doesn't use coping skills as learned. Ashiyah discussed with QMHP-C that she has difficulty with being dishonesty as she manipulates situations for her own benefit which causes trouble building healthy friendships. Ashiyah further discussed with QMHP-C, “I just don’t trust people”.
